ONLINE REQUIRED FORMS FOR REGISTERED CAMPERS
ALL registered camper's parent/guardians must complete and submit The University of Texas Medical and Transportation forms. These must be submitted PRIOR to arrival at camp, and no one can participate until they are on file.
Medical forms can now be completed using DocuSign, a secure document handler. Please click on the correct session for your camper (or first session if attending multiple sessions). DocuSign will automatically send your forms to our office. If you wish to print and complete forms, please forward them to the camp address below.
